4.2 Create Forms template

If you use Microsoft Forms as your survey system, templates are first created in Forms before they are added to ShareControl Transparency.

4.2.1 Create a survey template in Forms

  1. Open Forms by clicking on the link in the navigation menu in ShareControl Transparency or by opening https://forms.microsoft.com.

  2. Click New Form.

  3. Add a title and description.

  4. The first question has to be a Text question with the prompt:

    Please enter the e-mail address you received the survey invite for.

    An answer should be required for this question. This question is necessary for linking responses you get with the correct supplier in ShareControl Transparency.

  5. Click on the Add/insert new button to add a new question. You can select between multiple different question types where Choice and Text probably are the most common. Add all the questions you want.

A survey template in English and Norwegian created by Share Control is available in the app as an Excel file. Click on Resources in the navigation menu and download the “Survey Template for Forms” file. You can copy and paste these questions into Forms to use it as a survey.

4.2.2 Add survey template in ShareControl Transparency

  1. Open your finished form in Microsoft Forms.

  2. Click the “” button in the top right and then click on Collaborate or Duplicate.

  3. Click “Share the link to collaborate and view result”.

  4. Select “Specific people in (Your Company) with the link can edit and view result”.

  5. Share the form with the the ShareControl service account by entering its name or email address. The name is most commonly “Share Control” and the email address is often “svc_sharecontrol@yourcompany.com”.

  6. Click Collect responses.

  7. In the window that appears, select Anyone can respond and click the Copy link button. Do not shorten the URL.

  8. Open the ShareControl Transparency app and go to Settings > Survey templates.

  9. Click New in the toolbar at the top.

  10. Give the template a fitting name and paste the URL you copied in step 7 into the field Forms URL.

  11. Click Save & Close.

The template is now available. We advice to test a new template by first sending it to yourself in the app to make sure it works as expected. If you have any problems, contact us at support@sharecontrol.no.

4.2.3 Add a translation to a survey template (optional)

  1. Open the form in question in Microsoft Forms.

  2. Click the “” button in the top right and then click on Multilingual.

  3. Make sure the primary language matches the language used for the survey so far.

  4. Click Add additional language and select the language you want.

  5. Click on the language in the list. This will open a window where you can add translations for the title, description, every question, as well as multiple choice options.

  6. Click Back when you are done.
